PURPOSE:THE MISSOURI RURAL HEALTH INNOVATION RESEARCH CENTER, LED BY BOTHWELL REGIONAL HEALTH CENTER, WILL IDENTIFY, IMPLEMENT, AND EVALUATE SCALABLE "CARE-FROM-HOME" AND DIGITALLY ENABLED HEALTHCARE SOLUTIONS TO IMPROVE ACCESS, QUALITY, AND EFFICIENCY OF CARE IN RURAL MISSOURI.ACTIVITIES TO BE PERFORMED:1) DEVELOP A PLAN FOR IDENTIFYING A PILOT PROGRAM, WITH ALL STAKEHOLDERS; 2) SELECT APPROPRIATE TECHNOLOGIES FOR THE PILOT; 3) DO A CLINICAL AND OPERATIONAL INTEGRATION; 4) MONITOR AND CONTINUOUSLY EVALUATE.EXPECTED OUTCOMES:A PILOT DEMONSTRATING THE USE OF INFORMATION TECHNOLOGY FOR IMPROVING QUALITY OF RURAL HEALTH CARE.INTENDED BENEFICIARIES:THE MISSOURI RURAL HEALTH INNOVATION RESEARCH CENTER IS EXPECTED TO GENERATE MEASURABLE IMPROVEMENTS IN ACCESS, QUALITY, AND EFFICIENCY OF CARE ACROSS PARTICIPATING RURAL PROVIDERS. ANTICIPATED IMPACTS AND EVALUATION METHODOLOGY ARE GROUNDED IN BASELINE CONDITIONS DOCUMENTED IN BOTHWELL REGIONAL HEALTH CENTER'S 2025 COMMUNITY HEALTH NEEDS ASSESSMENT.SUBRECIPIENT ACTIVITIES:THE RECIPIENT DOES NOT INTEND TO SUBAWARD FUNDS.
